At the Community for Positive Aging we help seniors living at home who need assistance to maintain their independence.
We do this by:
Offering case management and options counseling services to adults, ages 60+ and to family caregivers.
Creating an outcome-based care plan to help seniors maintain independence by arranging for services such as housekeeping, bathing assistance, respite for caregivers, daycare, home-delivered meals, and transportation.
Helping coordinate and implement care with clients, service providers, and natural support networks.
Advocating for clients to access assistance from outside agencies, family members and volunteers.
Providing a holistic, strength-based assessment of each client’s needs to determine eligibility for assistance.
Case Management Services & Programs
There is no charge for most of our senior services. Funding for our programs comes from our partnership with Multnomah County’s Aging and Disability Services Division, grants from community partners and individual donations.
Options Counseling
We provide decision-making support to seniors and family members—at home, at the Center, or over the phone. We help individuals develop plans to address short- and long-term needs that align with the senior’s preferences, strengths & values. Follow-up support provided for up to 3 months.
Oregon Project Independence
Oregon Project Independence (OPI) assists elderly persons or persons diagnosed with Alzheimer’s or a related condition who need help with activities of daily living. Services include assistance with bathing, dressing, exercising, shopping, heavy housekeeping, medication reminders, and meal preparation by a certified home care worker. There is a sliding scale fee for this program based on income.
Family Caregiver Support Program
Caregivers help aging family members and friends stay in their home by doing chores such as shopping, housekeeping, cooking and providing personal care.
Caregivers face many challenges including physical and emotional stress, less time for their personal and family life, conflicts with work responsibilities and financial hardship.
We offer free assistance to caregivers providing care to those ages 60+ , caring for individuals with Alzheimer’s of any age, or grandparents caring for children.
We provide needs assessment, care planning, and coordination of respite and in-home services to caregivers. Call the Info & Assistance Specialist at 122 or email staff@communityfpa to learn more about the family caregiver program.
