Improving Resource Navigation, Education & Access
The Community for Positive Aging is committed to expanding our ability to address the needs of BIPOC and low-income seniors as well as adults living in subsidized housing.
To address disparities that low-income people of color face in utilizing and navigating health resources and systems, our multilingual Community Health team provides education, outreach, and one-to-one coaching around resources and services that help people live long, healthy, and meaningful lives
transportation to medical appointments
vaccine and communicable disease education and access
reducing social isolation
health system and care navigation
disability support
insurance access and navigation
We help our clients navigate issues such as:
preventative health measures
mental health resources
food access and nutrition education
active living
intergenerational opportunities
and emergency preparedness
